Proof of Licensure
The most up-to-date information concerning an occupational therapist’s licensure with NSOTR can be found in our Public Registry: Search for an Occupational Therapist. If you require additional proof of licensure, please see the below options:
Confirmation of Licence Certificate
Registrants can download a Confirmation of Licence Certificate through their online Portal. There is no fee to download this document. Please note that this Certificate is only current and valid as of the date of printing.
To download the document, on the main page of your online Portal, click on the “License” button underneath the “Active License” heading:

Regulatory History Form
This document is sometimes also called a:
- Certificate of Standing (COS)
- Verification of Registration (VOR)
- Professional History Form (PHF)
NSOTR will provide information about your registration, licensure, and professional history to other regulatory bodies upon request. Please note that NSOTR may not be able to provide all information which another regulatory body asks for if we cannot legally release certain information under Nova Scotian or Canadian law.
To request a Regulatory History Form, please complete the online Regulatory History request form through your Online Portal. If the other regulatory body has a specific document or form they would like you to use, you must complete and sign the “registrant” portion of that document (if relevant) and upload this document to the online request form.
There is no fee for this service.
Labour Mobility Support Agreement (LMSA) Form
If you want to become licensed with another Occupational Therapy regulator in Canada, you must request that NSOTR send that regulator a Labour Mobility Support Agreement (LMSA) Form and a Regulatory History Form.
To request an LMSA Form, please complete the online LMSA request form through your Online Portal. The other regulator will give you a copy of their LMSA form. Complete the first section of this document and then upload it to the online request form.
There is a $40 fee for NSOTR to send an LMSA Form to another regulator. Payment can be made online when submitting the request.
