Proof of Licensure

Confirmation of Licence Certificate
Registrants can download a Confirmation of Licence Certificate through their online Portal. There is no fee to download this document. Please note that this Certificate is only current and valid as of the date of printing.
To download the document, on the main page of your online Portal, click on the “License” button underneath the “Active License” heading:

Regulatory History Form
This document is sometimes also called a:
- Certificate of Standing (COS)
- Verification of Registration (VOR)
- Professional History Form (PHF)
NSOTR will complete documents from other regulatory bodies to confirm the licensure and/or professional history of a registrant. Please note that NSOTR may not be able to provide all information which another regulatory body asks for if we cannot legally release certain information under Nova Scotian or Canadian law.
There is no fee for completion of a Regulatory History Form. Please send the document you would like completed to registration@nsotr.ca. If the document includes a declaration granting NSOTR permission to provide information to a third party, please ensure that it is signed.
LMSA Form
If you want to become licensed with another Occupational Therapy regulator in Canada, you must request that NSOTR send that regulator a Labour Mobility Support Agreement (LMSA) Form and a Regulatory History Form. That regulator will give you their LMSA form and Regulatory History form to be completed by NSOTR.
Please send the document you would like completed to registration@nsotr.ca. There is a $40 fee for NSOTR to complete an LMSA Form, and NSOTR will invoice you directly upon receipt of the form.
