You can submit a complaint about an occupational therapist by clicking the “Submit a Complaint” button below. This will take you to an online form.

We will need:

• the name of the occupational therapist you are concerned about
• as much detail as possible about the nature of your concerns
• as well as your full contact information.

If you do not know the name of the occupational therapist, please review our online register here or contact us for assistance.

• You require assistance filing a complaint due to accessibility needs or a language barrier;
